Dear Community,

We invite you to take five minutes to participate in the Black Health Survey—a vital tool designed to help improve the health and well-being of Black residents across Colorado.

Your voice matters. By sharing your experiences and insights, you’re helping to shape programs, resources, and policies that better reflect the needs of our community.

We also encourage you to share the survey widely with your networks—friends, family, faith groups, nonprofits, and neighborhood organizations. The more voices we hear, the stronger and more accurate the data becomes.
